How do I schedule an appointment?

We do not do any insurance billing, but patients can seek reimbursement if they have out-of-network benefits. We are a fee-for-service practice and do not contract with insurance companies. Patients are responsible for full payment at the time of service and can use FSA and HSA funds for payment. We can provide a coded claim form (superbill) to patients seeking insurance reimbursement or for tax purposes.

Commercial insurances cover and offer the vast majority of our recommended lab tests. If patients have a high deductible insurance, oftentimes we are able to provide more affordable labs when our patients prepay for their labs outside of insurance (See discounted lab pricing for our patients here.)

Patients using credit cards pay a 3% fee convenience fee. Patients can avoid the fee by paying with Zelle, check, or cash.
